Learn How Scabies Spreads to Others

Understanding What Scabies Is and How It Spreads

Scabies is a skin condition caused by tiny mites called Sarcoptes scabiei. These mites are microscopic — so small you cannot see them without a magnifying glass. The mites burrow into the upper layer of human skin and lay eggs, which causes intense itching and a characteristic rash. Understanding how scabies spreads is important for preventing infection in yourself and others.

The disease spreads primarily through direct, prolonged skin-to-skin contact with an infected person. When an infected person touches another person's skin for an extended period, the mites can transfer. This is why healthcare providers consider scabies a contagious condition. However, the level of contagiousness depends on several factors, including how many mites are present on the infected person's skin and the length and nature of contact.

Scabies spreads differently than many other skin infections. Unlike fungal infections that can spread through brief contact, scabies typically requires more sustained contact. Research shows that a single handshake is unlikely to transmit scabies, but holding hands for several minutes or longer contact increases transmission risk significantly. Close family members, romantic partners, and healthcare workers who have frequent contact with infected individuals face higher risk.

The mites move slowly across skin. They can travel only a few centimeters per minute. This means they need time to move from one person to another during contact. Once a mite reaches new skin, it typically burrows within 15 to 20 minutes. After burrowing, it becomes much harder to remove through simple washing or casual contact.

Practical takeaway: Scabies requires sustained skin-to-skin contact to spread. Brief, casual contact like shaking hands or hugging for a few seconds carries minimal transmission risk. The highest-risk situations involve prolonged contact such as sleeping together, hand-holding for extended periods, or wrestling and play-fighting.

Direct Skin-to-Skin Contact as the Primary Transmission Route

The most common way scabies spreads is through direct skin-to-skin contact between an infected person and another person. This contact must typically last 15 minutes or longer for significant mite transfer to occur. Healthcare facilities report that scabies outbreaks in hospitals and nursing homes almost always start with direct contact between infected and non-infected individuals.

Within families, scabies spreads readily because family members have multiple opportunities for close contact. A person with scabies may sleep in the same bed as a partner, hug children, or have extended physical contact with household members. Studies indicate that 30 to 50 percent of household members of an infected person will also develop scabies within one to two months if no treatment is provided. This high transmission rate reflects the nature of family living arrangements.

Certain populations face higher transmission risk. Infants and young children have more frequent physical contact with caregivers. People living in crowded conditions — such as dormitories, shelters, or facilities for people with developmental disabilities — experience more skin-to-skin contact opportunities. Elderly individuals in nursing homes also have elevated risk because staff members may have prolonged contact while assisting with bathing, dressing, or physical care.

The infected area of skin matters for transmission risk. If a person has scabies primarily on their hands or forearms, and another person holds their hands for an extended period, transmission becomes more likely. If scabies is present in areas less frequently touched — such as the feet or buttocks — the transmission risk through casual contact decreases. A single mite transferred to new skin can potentially start an infestation in the new host.

Practical takeaway: Reduce transmission risk by avoiding prolonged skin-to-skin contact with people known or suspected to have scabies. If you must interact with an infected person, wash your hands thoroughly afterward and avoid touching your face. Healthcare workers should use appropriate personal protective equipment when caring for scabies patients.

Indirect Transmission Through Contaminated Surfaces and Shared Items

While less common than direct skin contact, scabies can spread indirectly through contaminated bedding, clothing, and shared items. Scabies mites can survive for 48 to 72 hours on fabrics and surfaces away from human skin. This means that bedding, pillows, mattresses, blankets, towels, and clothing worn by an infected person may harbor mites capable of causing infection.

Indirect transmission typically occurs when a non-infected person comes into contact with contaminated items and then transfers mites to their own skin. For example, a person might sleep on bedding previously used by someone with scabies, or wear clothing that belonged to an infected person. The mites must then burrow into the new host's skin to establish infection. The risk from indirect transmission is generally lower than from direct contact, but it remains a valid transmission route.

Research on scabies transmission suggests that indirect spread accounts for a smaller percentage of cases compared to direct contact. However, in certain settings, indirect transmission becomes more significant. In healthcare facilities, contaminated bedding that is not properly laundered can transmit mites to new patients. In households, shared beds, couches, and chairs used by infected family members present transmission risk. Shared towels used for bathing or showering carry some risk, though the risk is lower than with bed linens.

The viability of mites on surfaces depends on environmental conditions. Mites survive longer in humid conditions and may die more quickly in very dry environments. Temperature also affects survival. Mites are sensitive to heat, which is why hot water washing and high-temperature drying of contaminated linens effectively kills them. Mites cannot survive temperatures above 50 degrees Celsius (122 degrees Fahrenheit).

Practical takeaway: Wash all bedding, towels, and clothing worn by an infected person in hot water (at least 50 degrees Celsius) and dry on high heat. Items that cannot be washed should be placed in a sealed plastic bag for 72 hours to kill any mites. This prevents indirect transmission to other household members. Regular cleaning of surfaces is less critical for scabies prevention than thorough laundering of textiles.

High-Risk Settings and Populations for Scabies Transmission

Certain environments and populations experience higher rates of scabies transmission. Nursing homes and long-term care facilities rank among the highest-risk settings. The Centers for Disease Control and Prevention (CDC) notes that outbreaks in nursing homes can affect 30 to 50 percent of residents if not detected and treated quickly. These settings combine multiple risk factors: close quarters, frequent physical assistance from staff, shared bathing facilities, and populations with weakened immune systems or difficulty reporting symptoms.

Schools and childcare facilities also present elevated transmission risk. Children have frequent physical contact through play, wrestling, and games. They may share personal items and sleep on shared mats during naptime. Outbreaks in schools typically affect children in elementary grades more than older students. Parents of infected children often become infected through home contact.

Homeless shelters and overcrowded living situations create ideal conditions for scabies spread. People living in shelters share beds, blankets, and bathing facilities. Limited access to hot water and laundry facilities makes decontamination of bedding difficult. Social stigma may prevent people from seeking treatment, allowing the infection to spread further. Public health departments have documented large scabies outbreaks affecting hundreds of shelter residents.

Hospitals and medical facilities experience scabies transmission when infected patients are not isolated appropriately. Healthcare workers who provide direct care to scabies patients without protective measures can become infected and potentially transmit the disease to other patients. Immunocompromised patients, including those with HIV or on immunosuppressant medications, may develop a severe form called crusted scabies, which contains millions of mites and spreads more easily.

Military barracks, college dormitories, and other congregate living settings also experience scabies outbreaks. The combination of close living quarters, shared facilities, and populations in close contact creates conditions favoring transmission. Sports teams that engage in close physical contact, such as wrestling or rugby, have documented outbreaks among players.

Practical takeaway: If you live in or work in a high-risk setting and have symptoms of scabies — intense itching, especially between fingers, on wrists, or in skin folds — seek evaluation promptly. Early diagnosis and treatment prevent large-scale outbreaks. Healthcare facilities and congregate living settings should have clear protocols for identifying and isolating suspected scabies
